Important information that will help you with your day-to-day interactions with us:

  • Braven Health dedicated provider phone numbers

    • Behavioral health program: 1-888-444-0422
  • Braven Health has a separate Payer ID (84367)

    Providers must use this separate Payer ID for Braven Health for claims and other electronic transactions with Braven Health. If you haven’t already registered for Braven Health EDI, please do so immediately. If you work with a vendor or clearinghouse, please ask if they are registered with Braven Health for Electronic Data Interchange (EDI) services. Participation Information

We are committed to providing participating physicians and other health care professionals with the information and tools they need to help care for Braven Health members. If you currently participate in the Horizon Managed Care Network or the Horizon Hospital Network, we are pleased to advise you that you are also already participating for Braven Health members.

Your network participation status with Braven Health plans will be the same as your participation status in the Horizon Managed Care Network or the Horizon Hospital Network.

If you participate in the broad Horizon Managed Care Network or the Horizon Hospital Network, you are participating with:

  • Braven Medicare Choice (PPO)
  • Braven Medicare Freedom (PPO)
  • Braven Medicare Group (HMO-POS)
  • Braven Medicare Access Group (HMO-POS)

If you participate in the subset of the Horizon Managed Care Network or the subset of the Horizon Hospital Network, you are participating with:

  • Braven Medicare Plus (HMO)
  • The subset used by the Braven Medicare Plus (HMO) plan is the same subset used for our existing Horizon Medicare Blue Advantage plans.
